Health Outreach is looking for a registered nurse or social worker to join our committed countywide service for marginalised and vulnerable adults. Based in Ipswich, you will be supporting patients to access primary healthcare and social care.
The service is focused on working with adults with issues that marginalise them from mainstream services; engaging or re-engaging patients with health services and addressing social issues that may also be impacting on their health.
The main patient groups are:
- Migrant workers
- Asylum seekers and refugees
- Gypsies and travellers.
The service works to an outreach model engaging with patients in their own environment.
You will creatively work and liaise with other professionals to help those who are most vulnerable. Working with partner agencies in health and wellbeing, advice, support services and housing, your role will be to work with refugees under the government's Vulnerable Person Resettlement Programmes specifically leading on identifying patient need and linking patients with appropriate services.
Working as part of a large NHS trust, you will have the clinical governance and professional development structures to support you in your role. Good teamwork, the ability to build relationships with other professionals and good communication skills are essential. The skills you bring to the team will depend on your registration, general or mental health nurse or social worker, and will be part of the multi-disciplinary approach to working with our patients.
This is an opportunity to work intensely with hard to reach groups in a variety of ways, utilising an outreach model to identify as well as address patient needs. It is a dynamic and creative role within a supportive team.
