£20,903 (pro rata for part time hours)

An exciting opportunity has arisen for an Independence and Wellbeing Practitioner (IWP) to join the vibrant, established, and supportive Adult and Community Services Social Care Teams based in the west of the county.

Bury Town Adult and Community Services Social Care Team 

The Bury Town teamwork with complex and challenging cases including Care Needs Assessments, Carers Assessments, Reviews, Mental Capacity Assessments, and some Safeguarding/Welfare issues along with covering duty on a rota basis. In doing so, you will utilise your skills, promoting the person's strengths and potential for independence, reducing the need for more intensive services including prevention of hospital admission.

At the present time you will be working from home.  However, your base, once restrictions are lifted, will be the West Suffolk House office.
