Suffolk Family Carers is a respected charity that is passionate about delivering services to meet the needs of family carers of all ages. We provide a range of services to support adult, young adult and young carers across Suffolk.

Come and join the 'Connect for Health' programme and help benefit communities in the areas of Eye and North West, Stowmarket and South Rural (Shotley area). The Social Prescribing programme established in 2019 is funded and delivered in partnership with the Ipswich and East CCG. It is delivered alongside key Voluntary sector partners in other areas of Ipswich and East Suffolk

The Connect for Health Scheme supports people to improve their health and wellbeing, connect with their local community and find their own solutions.

Are you passionate about helping people to identify what matters to them? Are you looking for a new and exciting challenge, to utilise your health coaching / coaching skills? Do you enjoy developing communities to help reduce isolation and loneliness?

As a Senior Community Connector, you will work one-to-one with people in various settings such as GP practices, on board Suffolk Family Carers' mobile information centre or at their home (subject to Covid guidelines). You will have with the experience, skills and confidence to engage people and build positive links within local communities. You will also have experience of leading and supporting staff.

This role is part of a job share and your area of support will be based in the Stowmarket area, but working alongside the other Senior Community Connector to provide support to participants and staff as part of this programme.
