West Suffolk, Adult and Community Services.

Positions in either Bury Town Team or IWS (Independence and Wellbeing Service)

As an experienced qualified professional, you will act as a role model, playing a key part in promoting personalisation, independence, well-being and choice by providing a Social Care service to people which will include carrying out the most complex asset-based assessments/ reviews, short term enablement plans (STEPs), care and support plans, and safeguarding plans.

You will offer ongoing advice in line with the principles of SLCC / Signs of Safety and Wellbeing, promoting self-support, reablement / progression and personalisation. Where necessary, provide daily living equipment, adaptations, assistive technology, community and other resources to promote independence and wellbeing.

This is an exciting time for Adult and Community Services and we are currently recruiting following a redesign of our social care service, putting in place locality and partnership working and enhancing our integration with health. Our redesign keeps the residents of Suffolk at the centre of what we and you will do.

A relevant Social Work qualification and HCPC registration is essential, along with continued professional development. You will be required to travel frequently in rural areas, so you must hold a full driving licence and have access to personal transport.

The salary ranges is in line with our ACS Social Work Services Progression Scheme.

Please read the Job and Person Profile before applying, by uploading a CV and statement on what skills and experience you would bring to the role and then completing a short online application.

For an informal discussion only, please contact Tracy McCormick on (01284) 758451.
