Complex Specialist Dementia Team, Ipswich and East Area, Adult and Community Services.
As a professional Social Worker at Grade 5, you will act as a role model, demonstrating Suffolk County Council ‘ASPIRE values’ and a commitment to promoting the health and wellbeing of Suffolk residents; playing a key part in promoting personalisation, independence, wellbeing and choice.
As a member of a dynamic team, you will have an allocated case load, and support with duty responsibilities. You will be supported by experienced social workers and the Team Manager in the management of these through regular 1:1 and peer group supervision.
Adult and Community Services are enhancing our integration with health and the voluntary and community sector. This will include carrying out the complex asset-based assessments / reviews in line with the Mental Capacity Act 2005 and Care Act 2014. With a view to creating personalised short term enablement plans (STEPs), Care and Support Plans and Safeguarding Plans. Where necessary, you will provide support with assessment of daily living equipment including digital solutions, and adaptations liaising with occupational therapists throughout Adult and Community Services and health. You will also be instrumental in applications to Court of Protection for issues around finance, care and deprivation of liberty safeguards. You will liaise with primary and secondary health services on a regular basis in order to manage hospital discharges particularly from acute mental health wards and be proactive in admission prevention. You will work with statutory and voluntary agencies to enhance services offered to those with dementia and family carers in the community.
You will offer ongoing advice and support to families and individuals in line with the principles of Supporting Lives, Connecting Communities and the Signs of Safety and Wellbeing practice model. As such, promoting independence and wellbeing through enablement principles and personalisation of support packages, utilising social and community networks alongside more formal support networks within health and social care. You will offer specialist support and guidance to practitioners and professionals in the integrated neighbourhood teams with reference to dementia.
A relevant social work qualification and HCPC registration is essential, along with continued professional development. It is desirable that you have an interest in dementia and /or experience, in living or working with those experiencing dementia. Training will be available to support professional development.
You will be required to travel frequently in rural areas, so you must hold a full driving licence and have access to personal transport.
* Newly Qualified Social Workers start at £26,470. The salary ranges are in line with our ACS (Adult Community Services) Social Work Services Progression Scheme.
